P-Shot vs Penile Injections (Alprostadil and Trimix)

What is the difference between a P-Shot and penile injections such as alprostadil or trimix?

Intracavernosal injections of medicines such as alprostadil, or mixtures often called trimix, produce an erection when used before sex and are an established ED treatment. A P-Shot injects platelet-rich plasma from your own blood with the aim of supporting tissue repair, and it is experimental. They do different jobs, so they are not direct substitutes.

Key fact: The 2026 EAU guideline says intracavernosal PRP for erectile dysfunction should be used only in a clinical-trial setting. (European Association of Urology, 2026; page updated )

Frequently asked questions

Is a P-Shot better than penile injections?

They are not equivalent. Injections such as alprostadil are an established treatment that produces an erection when needed. The P-Shot is experimental and aims at longer-term tissue effects, with mixed evidence.

What is trimix?

Trimix usually means a mixture of three medicines (papaverine, phentolamine and alprostadil) injected into the penis to produce an erection. Preparation and regulation vary between countries.

Can I use injections and a P-Shot together?

A doctor may discuss combining approaches, but evidence for combinations is limited. Treatment is decided after assessment.

What are the risks of penile injections?

Pain, bruising, scarring with repeated use and prolonged erection (priapism). An erection lasting more than four hours needs urgent care.
